COMMUNITY PHARMACY PROGRAM

For Antrim, Benzie, Kalkaska, Leelanau, and Grand Traverse Counties.

This is a community-based program established to assist low-income older adult residents who have no prescription insurance coverage. The program will assist them in obtaining their prescription medications through the Patient Assistance Programs of major pharmaceutical companies.

For the homebound, a nurse is available for home visits.

Who is eligible?

• Residents of Antrim, Benzie, Kalkaska, Leelanau, and Grand Traverse Counties
• Sixty years of age or older
• Medications prescribed are for chronic medical conditions
• No prescription insurance coverage in place
• Unqualified for other programs
• Must meet income-based eligibility determined by the pharmaceutical company (usually less than $1,200 per month total household income)
