09 AREA AGENCY ON AGING OF NORTHWEST MICHIGAN: COMMUNITY CARE MANAGEMENT

1. Purpose: The goal of Care Management is to support independent living for the frail older adult, avoiding premature nursing home or adult foster care placement.

  1. are 60 years of age or older
  2. are unable to perform basic activities of daily living (i.e., personal care, household chores) allowing them to remain at home
  3. are experiencing several and/or complicated problems and require help in finding and arranging for services
  4. are medically eligible for placement in a basic or skilled nursing facility.
  5. or currently living in a nursing home and no longer need that level of assistance, but do need support to return to their home

8. Costs: The Care Management Program is funded by Older American Acts dollars, the Michigan Office of Services to the Aging, and local support. There is no charge for Care Management Services. Donations for services are suggested and welcomed. Any contributions received are used to provide additional service to Care Management clients.

09.a  AREA AGENCY ON AGING OF NW MICHIGAN
Medicare/Medicaid Assistance Program

1. Purpose: The MMAP Program offers insurance counseling to Medicare
beneficiaries and their families or representatives.

10 AREA AGENCY ON AGING OF NORTHWEST MICHIGAN: SENIOR COMMUNITY SERVICE EMPLOYMENT PROGRAM

1. Purpose: To assist individuals 55 and older in employment
and education.

11 BAY AREA TRANSPORTATION AUTHORITY (BATA)

1. Purpose: Door-to-door public transportation services throughout Leelanau and Grand Traverse Counties; scheduled fixed route service in Traverse City; non-emergency medical transportation services for Benzie, Grand Traverse and Leelanau Counties..

1.  Purpose:  Provide volunteer and bereavement services to the terminally ill and their families. Also, community outreach for grief and bereavement.

Foster Grandparents serve children (clients) identified and referred through volunteer referral stations. Children may have special emotional, physical, or intellectual needs that require additional love, patience, and individual attention to mature and function at their potential. Examples of special needs include children with developmental disabilities, mental impairments, learning disabilities, language barriers, sensory impairments, or victims of abuse and neglect.

1. Purpose: Senior Companions are older adults with limited income who understand the challenges and importance of maintaining independence later in life. Companions serve 20 hours each week and receive a small, nontaxable stipend and other benefits for their services.
